Student Forms
All children enrolled at MMO must have the following forms – signed by parents – in their student files at MMO. Discipline and Permission forms must be updated each year. Medical forms must be updated every two years and include a vaccination record. In addition, student files must contain a copy of the child’s passport or birth certificate, and a copy of the DCFS Standards – signed by a parent.

We are mostly full for the 2018/19 school year – please email [email protected] with your child’s birthdate and program preferences (Drop-In or Preschool, if Preschool – what days). If we have a spot available for your child, we’ll send you a link to register.
